This reservation request form is intended only for foreign-vaccinated dogs from high-risk rabies countries traveling to Rue's Kennels at LAX. Our team will contact you within 72 hours of submission. Please note: submitting this form does not guarantee a reservation. A reservation is only confirmed once you receive written confirmation from the CDC.. All dogs are required to travel with a completed CDC Certification of Foreign Rabies Vaccination and Microchip

High Risk Rabies Reservation Request

Please upload photos of your dog full body (standing facing foward) , & photo of your dogs face.

Please provide the pet passport books or vaccination certificates showing proof of rabies vaccination

Please provide Rabies vaccine titer lab work from CDC approved labs, if available. If no results are uploaded, we will assume you do not have Rabies vaccine titer for your dog.


If foreign-vaccinated dogs from high-risk countries do not have a valid rabies serology titer, the dogs must be quarantined for 28 days at a CDC-registered animal care facility after they are examined and revaccinated

For Customs and Border Protection release we require a photo of passanger / owner passport & Visa. (Please combine into one .Jpeg if sending both passport & Visa)
